We establish this relationship through responsive communication from the outset when you come in with your newborn baby for our new family consultation.
This initial meeting and discussion help us to best understand the needs of both you and your child. Enough time is allotted for you to ask all your questions and express any concerns you may have. We will provide you with good, honest health information.
Your chosen doctor will invite you to be a partner in your child’s ongoing care. Our main goal and mission is the safety and healthy growth of the young ones in our community.

CHC is composed of a diverse group of providers that work closely together to treat not only new and different symptoms but your whole child. Our team includes board-certified pediatricians, nurse practitioners, nurses, a pediatric nutritionist, and a special needs expert.
Parents are integral members of our team as well. We encourage their involvement and work closely with them to ensure the treatments we provide are effective, being complied with at home, and that your child is thriving between clinic visits.
Full Circle of Care
This is our model of comprehensive pediatric health care from birth through adolescence. Our 360-degree approach covers preventive and urgent care, on-site pediatric nutrition guidance, and specialist referrals when required.
We strongly believe our full circle of care gets your child to achieve his or her greatest potential. Our providers and on-staff specialists work cooperatively with parents to ensure the care that children need through each stage of development is met.
If referred to a specialist, we maintain continuous communication with them to monitor your child’s progress throughout.
The New Family Consultation
Parents are invited to meet and discuss all the critical subjects that most concern them.
Your chosen board-certified pediatric physician will take you from prenatal care to immunizations; from wellness checkups to specialty services such as pediatric nutrition counseling. They will talk about:
- The full circle of care model.
- Issues regarding breastfeeding, sleeping routines.
- Infant-safety and other care concerns.
- Specialty services including pediatric nutrition services, special care needs, and parent consultations.
- Schedule wellness visits and discuss immunizations
Behavioral and Child-rearing Consultations
CHC understands the role that parents have in teaching children. Good eating habits, behavior, mental influences, and language are learned by children at home from an early age. They say healthy habits start at home, but this can also be where bad habits start if certain practices are not in place.
Our doctors and nurse practitioners are available for consultations on all aspects of child-rearing. Common concerns include breastfeeding, toilet training, and learning or social difficulties at school that can lead to behavioral issues.
The appropriate time is allotted to address your child’s specific issue. We review all ways and means to resolve it, including referring you to specialists for further care if necessary.
Areas of concern include:
- Questions from pregnant mothers and their partners.
- Good eating habits at different stages of growth.
- Establishing good habits, learning techniques, and routines for toddlers.
- Managing siblings and peers.
- Starting school, homework, and learning issues.
- Out-of-school activities, games, or television.
- Pre-adolescent bodily and social changes.
- Impact of family events and crises on children.
The goals of these consultations are to explore and address the nature of these concerns and discuss how they can be dealt with. We give feedback and our professional opinions about your child’s needs and behavior. And decide together with you, the best options and how to proceed moving forward.
Patient-Centered Medical Home
The American Academy of Pediatrics (AAP) endorses patient-centered and family-centered care as the basic building block and the cornerstone of the medical home concept.
Our core values at CHC are compassion and comprehensive care focused on the developing child’s physical, mental, and emotional needs. Hence, we are in the process of becoming a Patient-Centered Medical Home (PCMH), where care is provided in a child-centered, family-focused manner, as our patients need it.
A PCMH provides a full circle of evidence-based care that is comprehensive, patient-centered, and accessible.
